Prescription Renewals

Medication refills * - $30

* 1) Applies to renewal requests received for patients without an
appointment, who have not been seen in office for > 3 months

2) All efforts will be made to provide appropriate refill amounts
when seen

3) If the medication is deemed urgent, an appointment will still be

required, with enough refill sent to cover the wait time and payment

collected when in office

Transfer of Medical Records

Fee applies to documents collected via paper, usb, email - $30

Travel appointment (per trip)

Initial consultation - $60

Follow up (for review or immunization) - $20
